Key Responsibilities

The Clinical Pharmacist will be responsible for performing a wide range of high-impact duties within the pharmacy and ICU departments, including but not limited to:

  • Verifying prescriptions and treatment sheets for ICU patients to ensure accuracy, appropriateness, and safety.

  • Contacting prescribing doctors to resolve prescription discrepancies and provide professional input for safer patient outcomes.

  • Reporting on therapeutic drug monitoring (TDM), especially for drugs with narrow therapeutic indices, and making dosage recommendations based on clinical parameters.

  • Participating actively in the Medicines and Therapeutics Committee, Infection Control Committee, and ICU Users Committee.

  • Contributing to the development of hospital-wide pharmaceutical policies, including antibiotic usage, chemotherapy administration, infection control, and disinfection procedures.

  • Assisting in designing and refining documentation tools such as prescribing templates and dosage calculation sheets.

  • Providing up-to-date drug information and clinical decision support to doctors, consultants, nurses, and other healthcare personnel.

  • Collaborating with doctors in creating personalized medication plans, particularly in complex cases involving renal, hepatic, or polypharmacy considerations.

  • Updating the pharmacy system database with current drug information, stock levels, expiry dates, and other relevant data.

  • Educating and counseling patients regarding proper medication usage, dosage instructions, possible side effects, and drug interactions.

  • Running statistical reports for the pharmacy department to track usage patterns, cost-efficiency, and performance indicators.

  • Participating in interdisciplinary committees and contributing pharmaceutical insights to patient care and safety strategies.

  • Interpreting lab results that affect medication planning, such as liver and kidney function tests.

  • Vetting and registering medications for approval before inclusion in the hospital’s formulary.

  • Monitoring and investigating medication errors and implementing preventive measures to improve workflow and reduce future risk.

  • Participating in ongoing training and professional development to keep pace with evolving pharmaceutical practices and technologies.

Qualifications and Skills

To qualify for this role, candidates must meet the following educational and professional criteria:

Education and Licensing:

  • A Master’s degree in Clinical Pharmacy from a recognized institution.

  • A Bachelor’s degree in Pharmacy from an accredited university.

  • Valid registration with the Pharmacy and Poisons Board of Kenya.

  • A current and active practicing license.

Professional Experience:

  • Minimum of three (3) years of experience as a Clinical Pharmacist in a hospital setting.

  • Demonstrated knowledge and practical understanding of ICU medication management, therapeutic drug monitoring, and pharmaceutical protocols.

  • Experience in hospital policy development, medication safety analysis, and interdisciplinary collaboration.

  • Strong knowledge of pharmacology, pharmacokinetics, and pharmaceutics.

  • Excellent communication, interpersonal, and problem-solving skills.
